Abstract

Objective: To assess whether tissue eosinophilia (≥10 eosinophils/hpf) is associated with higher endoscopic recurrence in patients with chronic rhinosinusitis with nasal polyps (CRSwNP) after functional endoscopic sinus surgery (FESS).

Methods: A retrospective observational cohort of 37 adult patients with CRSwNP who underwent FESS at the Central Military Hospital in Bogotá (2014-2022). Patients were divided into groups with and without tissue eosinophilia based on histopathology reports, and postoperative endoscopic recurrence was compared.

Results: The mean age was 47.9 years, with a male predominance (54.1%). Tissue eosinophilia was present in 70.3% (n=26) of patients. Endoscopic recurrence was remarkably high and comparable between both groups: 88.5% in the eosinophilia group vs. 90.9% in the non-eosinophilia group (p=1.000). The median time to recurrence was longer in the eosinophilia group (89 days) compared to the non-eosinophilia group (40 days), but this difference was not statistically significant (p=0.232). No significant differences were found in comorbidities such as asthma or ASA sensitivity.

Conclusion: In this Colombian cohort, tissue eosinophilia as defined by the EPOS 2020 cut-off (≥10 eosinophils/hpf) was not associated with a higher frequency of endoscopic recurrence. The high overall recurrence rates (~90%) and lack of statistical significance may be attributed to the small sample size. Larger prospective studies are needed to establish local predictive cut-off points for recurrence.
