Abstract

Background: Extranasopharyngeal angiofibromas are a rare variant of an already uncommon head and neck tumor that can originate from any cervicofacial structure and whose epidemiology, clinical behavior, and prognosis significantly differ from the classic type.

Clinical case: A 51-year-old male presented with two tumors originating from the right retromolar trigone and the right tonsillar fossa and underwent surgical resection of the lesions. Histopathological examination revealed bundles of smooth muscle, scarce adipose tissue, dilated and congested blood vessels with variable wall thickness, hemorrhagic fibrous stroma, and necrotic foci; suggestive of a nasopharyngeal angiofibroma. The patient recovered well from surgery, was discharged 24 hours after the procedure, and has not presented recurrent masses after a one-year follow-up.

Conclusion: Extranasopharyngeal angiofibroma is one of the less common head and neck tumors, with varied clinical manifestations depending on the site of origin, which can be successfully managed with surgical resection and low recurrence rates.
